First Citizens Junior Golf Tour

I understand and affirm that the First Citizens Junior Golf Tour (herein after known as FCJGT) and course owners and/or operators are not liable for any accidents or injuries suffered while participating in FCJGT sponsored events. By completing the registration process, I agree to fully indemnify and hold harmless the FCJGT and its agents, representatives and volunteers from any damages suffered by me, my family or my child while participating in, or traveling to or from any program sponsored by the FCJGT. Additionally, I agree that In the event of injury or illness to my child, I hereby grant authority to any physician to render such emergency medical treatment as the physician deems necessary under the circumstances. I authorize emergency transportation as necessary. I agree to be responsible for any charges incurred in the treatment of my child.

CONDUCT AGREEMENT: By submission of this application, I agree to the following: (1) I agree to abide by the regulations and instructions of the FCJGT and its representatives; (2) I agree that there are certain risks inherent in the game of golf and accept personal and sole liability for all such risks, including, but not limited to any health-related risks; (3) I agree that this application and entry into any tournament is subject to rejection at any time (including during a tournament) by the FCJGT. The reason for ejection may include inappropriate behavior.

I authorize the FCJGT to release my child’s scores and photographs to any newspaper, golf publication, golf website, golf organization or college golf coach. I have read and agree to the terms set forth.
